Help with a Federal Agency
Congressman Doug Lamborn and his staff may be able to assist you in obtaining information you need or resolve an issue you are experiencing with a federal agency. While a favorable outcome cannot be guaranteed, the office can often intervene on a person's behalf to answer questions, find solutions, or break through bureaucracy. Regardless of which agency you need help with my office will need a completed privacy authorization form to comply with the provisions of the Privacy Act to engage on your behalf.
My staff and I can only engage on behalf of residents of Colorado’s Fifth Congressional District. To confirm that you live in the district, click here.
Once all the required fields have been completed on the webform, you will receive a confirmation email. If you do not receive this email, please contact my office at 719-520-0055.
If your issue is with the IRS and you filed jointly, both parties need to fill out individual forms.
Please make sure to include all pertinent information and claim numbers in your correspondence, such as:
- Your Social Security number for a case involving Social Security;
- Your Medicare/Medicaid number for a case involving Medicare/Medicaid;
- VA claim number for a case with the Department of Veterans Affairs;
- Taxpayer identification number (Social Security number, if for an individual) for an Internal Revenue Service problem, etc.;
- Your address, home phone number, and daytime phone number (if different than home), so that we can obtain any additional information from you that might be necessary;
- Copies of any related documents or correspondence that you may have from the agency involved. (If Applicable)
Please Note: The Privacy Act of 1974 (5 U.S.C. § 552a) requires that Members of Congress or their staff have written authorization before they can obtain information about an individual's case.
